Customer self-service domain add

Currently operator has to manually add every new customer domain via the admin interface. Should let customer-admins add their own domains with DNS-based verification.

Done. Customer-admin clicks + add domain → fills form → gets TXT record to add at their DNS → clicks Verify now → status flips to active. Mail doesn't flow until verified. Includes 45-entry blocklist for high-value targets.
