Allow customers to enable/disable tagging per domain

Some customers want borderline spam delivered with [SPAM] in subject rather than quarantined. Should be a per-domain choice via the customer settings page.

Built. /settings page now has compact per-domain controls with tag mode toggle and advanced threshold editor. Tag mode off by default.
